Requesting an Appointment
If your need is urgent, please call (480) 545-1100 to request a same-day appointment. Requests are handled during regular business hours: Monday through Friday, 7:30a – NOON and 1 – 4:30p.
Canceling/Rescheduling an Appointment
From time to time schedules can change and we understand. Please call our office 24 hours in advance to let us know if you need to cancel or reschedule your appointment or you may be charged for the visit.
We may also choose to discharge a patient for habitual late cancellations and or rescheduling. We want to serve all of our patients in a timely and efficient manner and effective communication is the best way to accomplish this. If you show up late to your appointment, the appointment may be subject to cancellation.
Contacting a Physician After Hours
We have a 24-hour, 7-day-per-week answering service. Call (480) 545-1100. All emergent calls received after hours will be responded to as appropriate to the individual need of the patient. An after-hours fee may be charged to you for services rendered during non-business hours for non-emergent matters.
Renewing a Prescription
Prescription renewals are handled 8 a.m. to 5 p.m., Monday through Friday. Please allow at least 48 hours during the business week to accommodate your request, as each request has to be approved by a physician and all physicians are not in the office every day.

Paying your bill
We accept Visa and MasterCard for online and telephone payments.

Requesting a Referral
Please refer to our list of preferred specialists. Depending on your insurance, a visit to our office may be required before you see a specialist. Please contact your insurance company for the requirements of your particular plan.
Requesting Medical Records
Stop by any our office locations to sign a release authorizing the release of your records. Our offices are open Monday through Friday from 8 a.m. – 5 p.m. and seasonally on Saturdays from 8 a.m.– noon.
A fee may be charged for medical records.
